  4. Central Government Health Scheme Card.
  5. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
  6. Passport.
  7. Certificate of identity in Original signed by a Municipal Councilor.
  8. Aadhaar Card.
  9. Arm’s license.
  10. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
  11. Ex-Servicemen Contributory Health Scheme photo card.
  12. Certificate of identity in Original signed by a Member of Legislative Assembly.
  13. Certificate of identity in Original signed by a Member of Parliament.
  14. Ration card having photograph of the applicant.
  15. Election Card / Voter ID.
Address Proof (Any one):
  1. Water Bill less than 3 months old.
  2. Landline/Broadband connection bill less than 3 months old.
  3. Allotment letter of accommodation issued by the Central/State Government less than 3 years old.
  4. Post office passbook having address of the applicant.
  5. Passport.
  6. Certificate of identity in Original signed by a Member of Legislative Assembly.
  7. Bank account statement less than 3 months old.
  8. Consumer gas connection card or book or piped gas bill.
  9. Certificate of identity in Original signed by a Member of Parliament.
  10. Driving License.
  11. Latest property tax assessment order.
  12. Aadhaar Card.
  13. Election Card / Voter ID.
  14. Passport of Spouse (Husband/Wife).
  15. Certificate of identity in Original signed by a Gazetted officer.

Is PAN Card Reprint required?
Financial Institutions or Banks request a PAN Card as proof of PAN. If your PAN Card is stolen or misplaced, you can apply for PAN Card Reprint and present Reprint PAN Card.
Who has to sign the Reprint PAN Application?
The PAN Card Reprint Form must be signed by the PAN Holder. If he or she is unable to sign, a left thumb impression must be done on the PanReprint form.
