  3. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.
  4. Pensioner card having photograph of the applicant.
  5. Ex-Servicemen Contributory Health Scheme photo card.
  6. Certificate of identity in Original signed by a Member of Legislative Assembly.
  7. Certificate of identity in Original signed by a Member of Parliament.
  8. Ration card having photograph of the applicant.
  9. Certificate of identity in Original signed by a Gazetted officer.
  10. Arm’s license.
  11. Passport.
  12. Aadhaar Card.
  13. Driving License.
  14. Central Government Health Scheme Card.
  15. Bank certificate in Original on letter head from the branch(along with name and stamp of the issuing officer) containing duly attested photograph and bank account number of the applicant.
Address Proof (Any one):
  1. Credit card statement less than 3 months old.
  2. Passport of Spouse (Husband/Wife).
  3. Depository account statement less than 3 months old.
  4. Bank account statement less than 3 months old.
  5. Water Bill less than 3 months old.
  6. Certificate of identity in Original signed by a Gazetted officer.
  7. Certificate of identity in Original signed by a Municipal Councilor.
  8. Certificate of identity in Original signed by a Member of Legislative Assembly.
  9. Consumer gas connection card or book or piped gas bill.
  10. Landline/Broadband connection bill less than 3 months old.
  11. Election Card / Voter ID.
  12. Post office passbook having address of the applicant.
  13. Domicile certificate issued by the Government.
  14. Driving License.
  15. Latest property tax assessment order.
  16. Property Registration Document.
  17. Allotment letter of accommodation issued by the Central/State Government less than 3 years old.

  5. 10ᵗʰ or 12ᵗʰ Mark Sheet of recognized Board.
  6. Marriage Certificate.
  7. Photo identity card issued by the Central Government or State Government or Public Sector Undertaking.

  11. Affidavit sworn before a magistrate stating the date of birth.

Is PAN Card Reprint required?
Financial Institutions or Banks request a PAN Card as proof of PAN. If your PAN Card is stolen or misplaced, you can apply for PAN Card Reprint and present Reprint PAN Card.
Who has to sign the Reprint PAN Application?
The PAN Card Reprint Form must be signed by the PAN Holder. If he or she is unable to sign, a left thumb impression must be done on the PanReprint form.
